Submitting a claim is easy. Here’s how:

  1. Log in to EquitableHealth.ca and click “Submit a Claim”.
  2. Fill out the interactive health or dental claim form.
  3. Attach your receipt and click “Submit”.

  1. Download the Equitable EZClaim app from the App Store or Google Play.
  2. Fill out the interactive health or dental claim form.
  3. Take a photo of your receipt and click “Submit”.
