What to include when you submit a complaint

When submitting your complaint, we kindly ask that you include the following:

  • Your full name;
  • Policy number and / or certificate number and / or claim number;
  • The name of your Advisor (if applicable); and
  • Any details regarding your complaint, including steps taken to try and resolve it.

If you have a complaint regarding Equitable’s Accessibility Plan or Privacy Policy, please skip to Step 4.
